Advance Care Planning

To ensure we provide care honoring your medical wishes, Shenandoah Oncology encourages everyone to consider the process of advance care planning. Simply stated, advance care planning means planning in advance for your medical care in the event you are unable to speak for yourself. Advance care planning is a standard part of every treatment plan, even if your diagnosis is highly treatable.

Though the task of advance care planning may seem intimidating, we encourage you to think and reflect, learn about different life-sustaining medical procedures, decide who will make decisions on your behalf, talk about your wishes with loved ones and doctors, and record your choices on an advance directive form(s).

Your nurse practitioner and our licensed clinical social worker are happy to assist with advance care planning and answering your questions. They are a valuable resource available to you as you review your options to make informed decisions. You may also choose to make an appointment to speak with your nurse practitioner or our licensed clinical social worker to initiate your advance care plan. Our office has many resources to offer you and your family, just ask!
